I have a salesman's game form for giving away a De Luxe radio.  It says the manufacturer is

De Luxe Mfg. Co.

173 West Madison Street

Chicago, Ill.

They make a small mantle set with a chrome front with the model number of 405.  The set has the schematic pasted to the bottom.  I have the set and the sales game form.  Should it be included in this listing?

The set was made for De Luxe by Warwick Manufacturing.
